Community Paramedic/Mobile Integrated Health Attendant

Position Summary

The Crisis Center of Tampa Bay brings help, hope and healing to people facing serious life challenges or trauma resulting from sexual assault or abuse, domestic violence, financial distress, substance abuse, medical emergency, suicidal thoughts, emotional or situational problems. The Community Paramedic/Mobile Integrated Health Attendant (CP/MIHA) position reports to the Senior Director of TransCare Operations. The position is responsible for the continuous development, facilitation and patient contact management of the CP/MIH program through monitoring of health trends, integration efficiency models and data capture. Analysis of data based upon the goals and objectives of the CP/MIH program will be a critical function of the position responsibilities.

Duties and Responsibilities

  • Administer patient care consistent with department protocols and physician orders. Perform technical medical skills with a high degree of accuracy.
  • Analyze lab values, and integrate these values into the patient assessment and integration into clinical decisions.
  • Complete initial and ongoing patient assessments associated with health, environment, family support, financial, etc. and develop effective managed care coordination plan.
  • Coordinate health and behavioral health services for patients, including other community resources, as required.
  • Coordinate appointments and follow-up with physicians, pharmacists and hospitals.
  • Communicate effectively with associated health practitioners that are part of the CP/MIH integrated network of stakeholders and clinicians.
  • Accurately and promptly complete patient contact reports with all associated documentation necessary to facilitate plan of care for the patient.
  • In conjunction with patient assessments and contact records, formulate and analyze data pursuant to program goals and objectives.
  • Provide a caring, consultative personality with patience to comfort the patient, explain and answer queries concerning diagnosis, treatment and integration to other clinicians within the CP/MIH network.
  • Conduct patient education (as necessary), including but not limited to diabetes prevention/treatment, hypertension, Congestive Heart Failure (CHF), Chronic Obstructive Pulmonary Disease (COPD), falls assessment, injury evaluation, geriatric frailty visits, and nutrition.
  • Educate to assist with a safe living environment for patient without infringing on clients rights.
  • Consult and provide education to family members and other supports to ensure that care is on-going and that all client needs are identified and met.
  • Participate in educational and public information programs.
  • Provide training to personnel, as needed.
  • Communicate professionally and exhibit a positive demeanor with all individuals; strives to develop a strong working relationship with peers and those in the healthcare system.
  • Complete required initial and ongoing training/continuing education (CEU) courses, as required, to maintain current state and county certifications and licenses; maintain a current copy of licensure, registration and/or certifications with Human Resources.
  • Arrive for scheduled shift on time and ready to deploy. Attend meetings as requested.
  • Conduct pre-trip vehicle inspections to ensure sound vehicle operations; maintain an orderly vehicle and ensures the vehicle, equipment and medications are clean, stocked, organized and response ready.
  • Maintain thorough working knowledge of the vehicle and equipment, its function and use, and report defective equipment and supplies.
  • Maintain a neat and professional appearance at all times in accordance with Company policy and Standard Operating Procedures.
  • Adhere to all Company Policies and Procedures, TransCare Standard Operating Procedures and Medical and Protocol Manual; direct any questions or concerns to the Senior Director of TransCare Operations.
  • Perform other such duties as may be assigned by the Senior Director of TransCare Operations.

Education and Experience

  • Minimum of 4 years’ experience as an EMT/Paramedic or higher.
  • High school diploma or equivalent.
  • Must be 18 years of age.
  • Possess current Florida Paramedic license.
  • Possess and maintain Basic Life Support (BLS), Cardio Pulmonary Resuscitation (CPR), Pediatric Advanced Life Support (PALS) and Advanced Cardiac Life Support (ACLS) certifications.
  • Possess current Florida Class E driver’s license with acceptable driving history.
  • Possess current CPR Certification.
  • Attain PVDL license upon employment and maintain current license.
  • Successfully complete Department of Transportation physical examination, federal criminal background screening and 10 panel drug screening upon conditional offer of employment.
  • Successfully complete FEMA ICS 100, 200 and 700A and Baker Act Training within 14 days of employment.
  • Successfully complete Emergency Vehicle Operators (EVOC) training and test within 14 days of employment.

Knowledge, Skills and Abilities

  • Possess basic knowledge of anatomy and physiology required to perform the clinical requirements of patient assessment and evaluation.
  • Understand the influences of culture, environment and psychosocial aspects on the physiological disease progression and management of individuals experiencing health issues.
  • Possess knowledge of the impact of commonly prescribed and over-the counter medications and the importance of medication reconciliation and compliance with the standards established within the CP/MIH program environment.
  • Knowledge of Florida Chapter 401.
  • Knowledge of HIPPA requirements as it relates to patient confidentiality.
  • Knowledge of the Baker Act as it relates to TransCare Transportation Services.
  • Knowledge of EMS radio system and codes.
  • Possess superior verbal and written communication skills with the ability to effectively communicate with patients, community partners, clients, peers and management.
  • Understand the emotional and medical needs of patients. Knowledge of crisis intervention and stress response and methods to ensure personal well-being.
  • Possess knowledge of rescue and emergency medical equipment
  • Maintain skill in addition, subtraction, multiplication, and division of all units of measure

Physical Demands/Working Conditions

Physical Requirements:

  • Unimpaired Mobility: Must be able to respond quickly to ambulance calls, must physically be able to access patients located in difficult terrain and dangerous environments and locations, must be able to climb stairs and other structures, must be able to work in close and dangerous environments, such as without limitation, damaged vehicles and structures on fire, must be able to carry heavy equipment and patients.
  • Physical Abilities: Must be able to perform strenuous physical requirements, including without limitation, lifting and moving of heavy equipment and patients in a variety of environmental and dangerous conditions, perform CPR and other lifesaving maneuvers. Water rescues may require the ability to swim at basic survival levels, and other exhaustive physical demands in emergent situations.
  • Fine Motor Skills: Must be able to perform required medical skills and techniques, including without limitation, bandaging, splinting and establishing an IV, performing endotracheal intubation and other advanced airway techniques, manual defibrillation, pleural decompression.
  • Vision: Must have correctable vision that permits the safe operation of emergency vehicles, the reading of gauges, fine print and figures associated with emergent medical care and treatment; and color acuity to differentiate varying colors that may be necessary in providing emergency care and treatment.
  • Hearing: Must be able to hear sufficiently to obtain information provided by patients, family members, bystanders, medical professionals and other emergency personnel; and adequate hearing necessary to provide emergency care and treatment in active, distracting environments.
  • Speech: Must be able to speak and enunciate clearly and at a level audible to others in loud conditions. Must be able to verbally communicate in stressful and distracting environments with patients, family members, medical professionals and other emergency personnel.

Working Conditions: Duties are performed primarily in a driving environment. Operates company vehicles designed with seats, benches, stretchers and/or wheelchair lifts. Occasional exposure to hazardous conditions and to blood/body fluids. Employee regularly works outdoors

Travel: Travel is limited to local day travel; primary functions are completed in an ambulance, medical transport vehicle or in a medical facility.

Hours: TransCare is open 365 days/year, 24 hours/day. Monday - Friday 8:00am - 5:00pm. Employee should be available and willing to work varying shift hours as assigned to include nights, weekends and holidays. Employee may be re-called to duty in the event of a declared disaster or emergency.
